Under the Texas Public Information Act, §6252-17a et seq., I am requesting an opportunity to inspect or preferably obtain copies (electronic preferred, but paper acceptable) of the following public records:

All documents related to the formal six-year evaluations of all medical school department chairs conducted during the years 2000-present as detailed in the policies stipulated in UTHSC-H HOOP 184 "Evaluation of Academic Administrators" and The University of Texas System Board of Regents' Rules and Regulations, Series 31101 "Evaluation of Administrators.”  Please include all documents related to the required consultation with the Faculty Senate regarding development of the review policies and procedures for the reviews; the actual policies and procedures for the reviews; the membership of the review committees; the required input from faculty, staff, and students to the review; the committee’s report; and the final written summary of the report submitted to the Executive Vice Chancellor for Health Affairs.
