I. POLICY AND GENERAL STATEMENT

Employees and trainees at The University of Texas Health Science Center at Houston (“University”) provide clinical care in a variety of settings, including University-based clinics, UT Physicians (the clinical practice of the University’s McGovern Medical School), UT Dentists (the clinical practice of the University’s School of Dentistry), UT Health Services, local hospitals and the Harris County Psychiatric Center (“HCPC”).  The purpose of this policy is to ensure that each patient’s healthcare information and appropriate charges for clinical care are thoroughly and timely documented in the applicable Electronic Health Record (EHR).

II. DEFINITIONS

Electronic Health Record (EHR): a paperless record of health-related information on an individual patient that is created, gathered, managed, and consulted by authorized health care clinicians and staff within a healthcare organization.

III. PROCEDURE

All employees and trainees providing clinical care at any site must adhere to the applicable policies in Appendix A (see below).
