This cutting-edge initiative is redefining healthcare delivery by leveraging technology to enhance patient outcomes, streamline care, and address critical gaps in the healthcare system.
The clinic is already making a meaningful impact, thanks partly to the leadership of Stephanie Garza, FNP, a Family Nurse Practitioner who brings a wealth of experience and a patient-centered approach. In preparation for her role, Garza actively shadowed the neurotrauma clinic team to understand the challenges patients face during hospital discharge and the transition of care.
Her involvement in daily multidisciplinary rounds has been instrumental in identifying systemic gaps in the discharge process—such as unclear care instructions, insufficient follow-ups, and lack of proper medication reconciliation—that are often the root causes of preventable hospital readmissions.
Stephanie’s dedication to understanding the patient journey—from admission to discharge—demonstrates her commitment to making a difference in transitional care. Her hands-on approach ensures that our virtual clinic addresses real-world challenges and provides solutions that improve continuity of care.
