UTEAP Providers

The staff at UTEAP would like to extend an invitation to you to join our EAP Preferred Provider Network. UTEAP provides services throughout the United States and is currently accepting applications from providers in most geographic areas. You are welcome to contact our Provider Relations staff at (800) 346-3549 to determine if we are recruiting in your area.

Our reimbursement rates are competitive.

Upon completion of EAP assessment services, you will only be expected to complete a short Client Record and Billing Form. This form serves the dual purpose of clinical documentation of the case and billing for your services. No separate billing forms are required and you can expect to receive payment in a maximum of four weeks.

Who we contract with/Credentialing Requirements
1. Licensed as a LCSW, LPC, LMFT, LCDC or licensed Psychologist.
2. All Clinicians must be licensed by the state for independent practice and possess a current professional license without restrictions, conditions or other disciplinary action
3. Have an absence of exclusions or debarment from participation in Medicare, Medicaid or other state or federal health care program
4. Have an absence of malpractice lawsuits, judgments, settlements or other incidents that indicate a competency or quality of care issue
5. Maintain Malpractice Liability Insurance with limit of $1,000,000 occurrence/$3,000,000 aggregate
EAP Overview

WHAT IS AN EMPLOYEE ASSISTANCE PROGRAM (EAP)?
The EAP is a service provided by employers (client-companies) for their employees and covered family members. EAP services are designed to assist employees in resolving their personal problems before work performance is negatively influenced. The EAP provides assessment, referral and short term counseling. The EAP protects and maintains the privacy & confidentiality of employee/client information consistent with HIPAA and other privacy laws.
WHAT AN EMPLOYEE ASSISTANCE PROGRAM (EAP) DOES NOT PROVIDE:
The EAP is not meant to replace an employee’s mental health or chemical dependency coverage through his/her health plan. The EAP is not a treatment program for chronic problems and does not provide school testing, psychiatric testing or any psychological testing. Testing and Diagnostic services are beyond the scope of the EAP. If the clients issue(s) cannot be resolved within the available number of EAP visits, then the EAP counselor should refer the client for longer term counseling or therapy services through their health insurance benefit plan or other available community resources.

How do I change my information or address?
Billing Questions

Upon completion of EAP assessment services, you will only be expected to complete the Client Record and Billing Form. The form serves the dual purpose of clinical documentation of the case and billing for your services. No other billing form is required. The Client Record portion of the form asks you to record the client’s diagnosis, treatment plan and disposition, along with brief demographic information. The Case Record is then forwarded to UTEAP for case closure and payment of your services. Once the Case Record is received by UTEAP, you can expect to receive payment within a maximum of four weeks. After submission of the paperwork, the case will be closed. We will not pay for additional sessions under this authorization once you submit your paperwork.

The EAP Client Record and Invoice forms are sent to you on the day of Authorization and are the primary documents you will complete when serving UTEAP clients. Please submit the Client Record and Invoice AFTER your last session with the client. All documents must be submitted to ensure payment in a timely manner. Fax billing forms to 713 500-3330.

Please be advised under the terms of the Provider Agreement with UTEAP, your reimbursement rate will be decreased as follows if there is a delay in returning the assessment/invoice forms:
• More than 60 days after the last session – 25% penalty
• More than 90 days after the last session – 50% penalty

Referral Process

The client contacts UTEAP to utilize services. Once our UTEAP staff gathers initial demographic and related information, the client will be sent a list of Network Providers that may meet their specific needs within a 10 +/- mile radius. The Client is instructed to review the list and contact any or all of the providers on the list to ascertain their availability and identify that they are using their EAP benefits. There may be certain instances when a client is referred to a particular provider by our Clinical staff. In these instances, the Clinical staff will contact the specific provider directly to discuss and confirm availability.
