Former Student Information and Forms
Former students who have completed their program requirements and have requested or been approved for Optional Practical Training (OPT) or STEM Optional Practical Training (STEM OPT) must follow the reporting requirements as outlined in the forms below.
These forms must be completed and submitted to OIA within 10 calendar days of any reportable changes. Please read the information at the top of the form for a detailed list of which changes are necessary to report.

Former students on OPT or STEM OPT must also submit a Travel Request Form any time they wish to travel outside of the U.S. and re-enter using their F-1 visa.
Please note that Travel Signatures are only valid for up to 6 months once the student has graduated.
