Web-Based Support Program for AD/ADRD Caregivers of Hospitalized Veterans Living with Dementia to Prevent Elder Mistreatment during Care Transitions from Hospital to Home
We propose to evaluate the feasibility and preliminary impact of Hospital GP4C (GamePlan4Care), a web-based support program, when implemented at the key inflection point of transition to home. We will examine effects of the intervention on: (1) key mechanisms related to elder mistreatment risk; and (2) elder mistreatment incidence in Veterans with Alzheimer's and related diseases. This proposal provides a clear path to future National Institute on Aging behavioral intervention Stage 2-4 projects (e.g., follow-on randomized clinical trials in Veterans and non-Veterans).
