- OIA Travel Acknowledgement Form (not for UTHealth Houston Sponsored F-1 Students or J-1 Physicians sponsored by ECFMG. See appropriate form below)
- J-1 ECFMG Travel Acknowledgement Form (J-1 Physicians sponsored by ECFMG are required to use this travel form)
