The University of Texas Health Science Center at Houston has partnered with Credential Solutions, Inc to streamline the transcript ordering process. Effective May 11, 2021, students and former students can request transcripts 24/7 to be delivered in electronic (PDF) format or mailed hard copy. Once a request is submitted, notification emails are sent both upon receipt as well as upon completion of the order. There is no charge for an official transcript, however a processing/delivery fee may be applied. FedEx delivery is also available at an additional charge.
We will no longer accept any mailed, faxed or emailed transcript requests.
Alert: Due to COVID-19 the US Postal service is experiencing significant delays with mail delivery within the United States and Internationally, with some countries not accepting mail for certain periods of time. To help ensure your official transcript is delivered in a timely manner, please request your official transcript to be sent electronically when possible.

If you attended UTHSC-Houston prior to 1991, you may not be able to order an electronic version of your transcript. Please allow for additional processing time.

If there is a hold on your record, you will receive an email with additional information. Please be aware that the Office of the Registrar cannot remove or explain holds placed by other offices.
Coursework transferred to The University will not be included on official transcripts. If you need copies of transcripts from other schools, please contact them directly.
If you are an investigator placing an electronic order, please attach a letter of release hand-signed by the student being investigated (copies/scans of original signatures are accepted) and a copy of your credentials. This letter will be kept for our records. If you are needing to pick up a hard copy, please email email@example.com for an appointment.
Transcripts can still be picked up in person at the Registrar’s office. Pick up is by appointment only. Please email firstname.lastname@example.org to schedule a time for pick up. If you are authorizing another individual to pick up your transcript, you will need to indicate the name of the individual in your request and submit a signed letter with the name and date of birth of the designated individual. Transcripts will be released with a valid driver’s license or government issued ID.
